Job summary

You will assist the care team nurse and provider on cases, research referral information, and serve as a resource to members and families with case updates. Responsibilities include case management, coordinating with providers, updating the case system, reviewing medical records, and delivering timely, clear reports.

Qualifications

• High school diploma or equivalent.
• Minimum 2 years’ experience in professional work environment.
• Proficient in Microsoft Word, Excel and Outlook.

Responsibilities

• Case management (55%+) and documentation in the case management system.
• Collaborate with providers to prioritize case management and coordinate care.
• Collect, process, and manage medical records relevant to each case.
• Provide regular case updates to physicians, nurses, members and leaders.
