Circle of Life Home Care Anishinaabe in Rosebud, SD is hiring a Case Coordinator for a full-time role dedicated to coordinating client care and onboarding Personal Care Attendants. The position emphasizes accurate record keeping and a compassionate, client-first approach to help clients stay independent at home.

You will collaborate with clients, PCAs, and the Regional Director, manage schedules and training, and support incident reporting and audits.

Qualifications

• Associate degree or equivalent required.
• Proficient with Microsoft Office applications including Outlook, SharePoint, Excel, and Teams.
• Strong time management and ability to meet deadlines.

Responsibilities

• Process PCA new hire applications and assist in onboarding.
• Conduct monthly home visits and wellness visits as needed.
• Manage new client inquiries, assessments, and requests.
• Maintain accurate system records and keep employee files current.
• Create schedules for Personal Care Attendants per care plan.
• Communicate with care providers, clients, and PCAs to provide service updates.
• Report incidents and follow up on escalated complaints.
• Supervise PCAs by setting attendance, performance, and conduct expectations.
• Set up training for new/current employees.
• Assist in caseload audits and respond to audit requests.
• Ensure office workspace remains professional and organized.
• Participate in weekly PCA/Client meetings with Regional Director.
• Attend community outreach events.
