Job summary

Circle of Life in Fort Yates, ND is seeking a Case Coordinator to support PCA onboarding, client assessments, and overall coordination of care. You will help maintain records and ensure timely service delivery in the local community.

Qualified candidates will have an associate degree or equivalent and strong computer skills (Outlook, Excel, SharePoint, Teams). Ability to manage time and work under deadlines while showing compassion is essential.

Qualifications

• Associate degree or equivalent from two-year college or technical school, or 1+ year of related experience.
• Knowledge of computer systems including Microsoft Suite with Outlook, SharePoint, Excel, and Teams.
• Ability to manage time, handle stressful situations, and meet deadlines.
• Willingness to assist and honor those we serve.

Responsibilities

• Process PCA new hire applications and assist onboarding.
• Conduct monthly home visits and wellness checks as needed.
• Process new client inquiries, assessments, and requests.
• Maintain accurate records for employees and clients.
• Create PCA schedules per client care plans.
• Communicate updates with care providers, clients, and PCAs.
• Report incidents and follow up on escalated complaints.
