Circle of Life in Fort Yates, ND is seeking a Case Coordinator to support PCA onboarding, scheduling, and client care coordination. The role involves regular client and PCA interactions, record-keeping, and collaboration with the Regional Director.

The ideal candidate will have an associate degree or equivalent, strong computer skills (Outlook/SharePoint/Excel/Teams), and the ability to manage time under deadlines while serving the community with empathy and professionalism.

Qualifications

• Associate degree or equivalent from two-year college or technical school, or equivalent combination of education and experience.
• Proficient with Microsoft Office (Outlook, SharePoint, Excel, Teams).
• Ability to manage time, handle stressful situations, and meet deadlines.
• Willingness to assist, encourage, and honor those we serve.

Responsibilities

• Process PCA new hire applications and assist in onboarding.
• Conduct monthly home visits and client wellness checks.
• Process all new client inquiries, assessments, and requests.
• Maintain accurate system records and current employee files.
• Create schedules for PCAs per care plans.
• Communicate updates with care providers, clients, and PCAs.
• Report PCA or client incidents and follow up on escalated complaints.
• Supervise PCAs by setting attendance and performance expectations.
• Set up training for new/current employees.
• Perform caseload audits and respond to audit requests.
• Keep the office environment neat and professional.
