Job summary

Adjoin in San Diego is seeking a Housing Services Specialist to collaborate with Veterans and the team to identify and secure affordable housing. You will deliver direct services to Veterans and their families who are homeless or at imminent risk.

Responsibilities include transporting Veterans to appointments, maintaining case notes, and developing Housing Stability Plans to help achieve permanent housing. The position requires a Bachelor’s degree or two years of equivalent experience, a valid

Qualifications

• Bachelor’s degree or two years of equivalent experience in working with veterans or homelessness.
• Valid driver’s license and insured vehicle.
• Must pass pre-employment drug screen and background check.
• Ability to travel within and throughout the counties served.
• CPR/First-Aid certification or completion within 45 days of hire.
• Must successfully complete Adjoin training programs.

• Complies with all safety standards and practices. Transport Veterans to appointments, housing searches, or other necessary locations, ensuring safety and timeliness during travel.
• Maintain case notes in an accurate and detailed manner. Maintain positive and professional communications and relationships with Veterans/individuals, household members, and collateral contacts.
• Provide direct services to a wide range of Veterans and their families that are homeless or at imminent risk.
• Collaborate with Veterans and other team members to identify and secure affordable housing.
• Manage all paperwork and meet all coinciding deadlines.
• Completes all training within required deadlines.
• Develop Housing Stability Plans (HSP) and action steps to achieve and maintain permanent housing for Veterans.
• Provide ongoing referrals and resources to Veterans throughout their time in the program and document on Referral Log and case notes.

Overview

Individual Veterans may have mental health diagnoses, physical disabilities, substance abuse challenges and other factors that may arise.

• Maintain case notes in an accurate and detailed manner. Maintain positive and professional communications and relationships with Veterans/individuals, household members, and collateral contacts.
• Provide direct services to a wide range of Veterans and their families that are homeless or at imminent risk.

Requirements

• Experience 18 years of age or older. Successful completion of pre-employment drug screen and background check.
• Travel Ability to travel within and throughout the counties in which services are provided. Ability to travel in and throughout the state on a quarterly basis to attend meetings, trainings, etc.
• License Insured, registered vehicle in safe working condition, and proof of current driver’s license.
• Compliance Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
• Degree Bachelor’s degree or two years of equivalent experience and/or educational equivalent in working with veterans and/or with individuals who have experienced homelessness and/or are at imminent risk of homelessness.
• Training Must successfully complete Adjoin’s training programs as assigned.
• Certification Certification in an approved First-Aid and CPR class; or successfully complete an approved First-Aid and CPR class within 45 days of the date of hire.
