Villa Esperanza Services in Pasadena is seeking a Case Coordinator responsible for coordinating services for individuals with developmental disabilities. The role includes tasks such as managing client referral packets, conducting orientations, and ensuring compliance with regulations.
This position offers a starting pay of $24-$25 per hour, with a schedule of Monday to Friday from 8:00 a.m. to 4:30 p.m. The ideal candidate should hold an Associates degree, have communication skills, and pass background checks.
Job Description: The Case Coordinator is responsible for coordinating services supporting individuals served. Assists with the effective management of the program case files, ensuring person-centered planning and compliance with agency and regulatory standards. This role serves as a liaison between supported individuals, families, staff, and external service providers to promote quality care and positive outcomes.
Essential Duties and Responsibilities:
Reviews, updates, and creates new client’s referral packets.
Determines new client’s eligibility and approves admittance in the program.
Maintains waiting list.
Conducts orientation/intake process for new clients.
Establishes suitable goals, objectives, strategies and services for clients.
Review’s program staff notes and related behavior charts.
Schedules and attends IPP/ISP meetings.
Updates client’s information annually.
Process and write Special Incident Reports (SIR’s) and informs all necessary internal external parties (Officer of the day, regional center, etc.)
Records and files outcomes of special incident reports.
Complete and process client’s termination documents.
Supervise, train, hire and discipline program staff.
Responsible to assist with daily program management.
Assist with program quality assurance and program evaluation as well as ongoing program assessments.
Assist with compliance for Federal and State regulations.
Assist with proper implementation of program design.
Develops community resources to include activities for clients and volunteer opportunities.
Supports ongoing efforts of program growth and expansion.
Ensures that program funds and expenses are within budget guidelines.
Responsible for the determining client’s suitability for program and/or need for discharge.
Villa’s Adult Day Program (ADP) offers a variety of life enhancing experiences for individuals with developmental disabilities and other disabilities, such as Alzheimer’s disease. This integrated model provides a safe, positive, caring alternative to nursing home care for those who do not need 24-hour skilled nursing. The program is designed to help people stay mentally and physically active, reduce isolation, improve their health, and prevent decline of their abilities. It is a community-based group program that supports families by providing a variety of social and health services to the person needing assistance. The program also provides Licensed Vocational Nurses to help meet the medical needs of our ADP participants and offers therapeutic exercises, nutritional snacks and access to occupational and speech therapy consultation.
Compensation: $24.00 - $25.00/hour
Schedule: Monday – Friday: 8:00a - 4:30p (35 - 40 hours/week)

Requirement/Qualifications:
Associates required, BA preferred.
Some familiarity or experience with medical terminology would be ideal, medical assistant/cna even with some writing ability and maturity.

Satisfactorily pass a background fingerprinting clearance from DOJ/FBI Community Care Licensing/Dept. of Social Services.
Satisfactorily complete a physical & TB screening.
