Job summary

Always Compassionate Home Care seeks a Case Coordinator to support the coordination team. You will work directly with our caregivers to ensure clients receive compassionate care, emphasizing scheduling and ongoing communication with clients and caregivers.

The role requires 3–5 years in home care, strong organizational and communication skills, and the ability to multi-task in a fast-paced environment. Bachelor’s degree preferred; on-call after hours.

Qualifications

Responsibilities

• Staff personnel according to home care patient schedules.
• Document and report changes in client status to Case Manager.
• Use time efficiently in a fast-paced environment.
• Multitask and adapt to dynamic caregiving needs.
• Be technically savvy with smartphones, tablets, and laptops.
• Communicate relevant information to clients, families, and coordinators in a timely manner.
• Maintain a daily call log and monitor the call dashboard.
• Follow up with clients and families regarding schedule changes and service needs.

Always Compassionate Home Care is seeking a Case Coordinator to work with our coordination team. The coordinator's primary responsibility is to work directly with our caregivers to ensure they are providing our clients with the compassionate care our clients deserve. Coordinators focus on delivery of care through scheduling and continuous communication with client's and caregivers.

Qualifications

• Working knowledge of HHA Exchange
• Excellent organizational and time-management skills
• At least 3-5 years of experience in a Home Care setting
• Great communication and interpersonal skills
• Attention to detail with problem solving skills

• Staffing open cases and providing daily support to clients and caregivers

Requirements

• Staff personnel according to home care patient's schedules
• Documents and reports to Case Manager when there is a change in client status, change in service requirement, change in aide assigned to case, client, nurse incidents, and complaints.
• Ability to use time efficiently
• Ability to work in a fast-paced environment and demonstrated ability to multi-task
• Technically savvy - proficient in use of smartphones, tablets, laptops, and other devices
• Exceptional communication skills - demonstrated ability to communicate and follow-up in a timely manner
• Maintains a daily call log and monitors the call dashboard
• Communicate all relevant information to client family and client service coordinators as needed
