Job summary

UnitedHealth Group is seeking a Home and Community Based Services Case Coordinator to oversee services for a caseload of clients daily. You will interface with clients, families, and staff to ensure quality care and accurate records, while coordinating daily scheduling and staffing needs.

The role involves payroll/billing support, client visits, and ensuring compliance with program requirements, with minimal onsite supervision.

Qualifications

  • Manage staffing operations, including hiring, orientation, and on-call rotation.
  • Assist with payroll, billing, and data accuracy by coordinating, entering, and auditing weekly payroll/billing and maintaining records.
  • Conduct and support client consultations and on-site visits, monitoring customer satisfaction.
  • Ensure compliance with company policies and program requirements, pay rate guidelines, overtime approvals, and incident reporting.

As the Home and Community Bases Services Case Coordinator, you will oversee the services provided to a caseload of clients on a daily basis. This responsibility includes interfacing with clients/families/employees to ensure quality service delivery. You will also be responsible for day-to-day staffing of cases, problem solving, assisting in the hiring process, providing orientation to new caregivers, payroll/billing, and ensuring customer satisfaction. As the Case Coordinator must be able to function with minimal onsite supervision and may perform some or all of the duties listed below.

Primary Responsibilities:

  • Coordinate daily client case scheduling, ensuring appropriate caregiver coverage and ongoing communication with clients and families
  • Manage staffing operations, including anticipating workforce needs, supporting hiring and orientation, and participating in on‑call rotation
  • Assisting with payroll, billing, and data accuracy by coordinating, entering, and auditing weekly payroll/billing and maintaining client and employee records
  • Conduct and support client consultations, on‑site home and supervisory visits, and monitor customer satisfaction, escalating issues as needed
  • Ensure compliance with company policies, state and funded program requirements, pay rate guidelines, overtime approvals, and incident reporting

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $19.00 to $22.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
