JOB SUMMARY:
Responsible for assistance in all programmatic areas, as well as case assistance for older adults, ages 60+, who are enrolled as OAC and NORC participants.
WHAT YOU’LL DO:
- Provide case assistance, including initial intakes, benefits/entitlements, information/referrals, advocacy, immigration services, and other case assistance as needed
- Writing comprehensive, accurate, and timely case notes
- Data entry into DFTA computer software program
- Completing and submitting monthly unit logs for case assistance and information/referral
- Assist with promoting and implementation of all department programs, as needed
- Responsible for transportation intakes and maintaining monthly unit service logs for participants
- Responsible for compliance to DFTA standards, including unit mandates
- Assist with review and entry of units, as needed
- Updating client online profiles as required
- All other projects assigned by Supervisor
CORE COMPETENCIES:
- General case assistance, benefits and entitlements, work with older adults, tracking, reporting, program assistance, adhering to all required deadlines
MINIMUM REQUIREMENTS:
- Experience working with constituents on a one-to-one basis; experience working with active adults a plus; Spanish speaking a plus
SALARY/HOURS:
Salary: $42,000
