Qualifications

Responsibilities

• Conduct detailed intake assessments and psychosocial evaluations.
• Develop individualized treatment plans in partnership with patients and the care team.
• Provide individual counseling focused on recovery and relapse prevention.
• Deliver program orientation and patient education.
• Monitor substance use through supervised toxicology screening.
• Deliver case management including discharge planning and referrals.
• Collaborate with nursing, medical, and clinical staff to support outcomes.
