Job summary

UnitedHealth Group's Lifeline Home Health Care is seeking a dedicated Physical Therapist to provide hospice services in home settings. You will assess patients, develop home exercise programs, instruct families, and document outcomes per hospice standards.

Qualifications include active state licensure, CPR, and the ability to work in diverse home environments. Valid driver’s license/transportation required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Able to function in home settings with diverse clients.
• Driver's license and access to a vehicle or public transportation.
• Ability to manage multiple tasks simultaneously.
• Strong communication, writing, and organizational skills.

Responsibilities

• Initiates physical therapy evaluation according to physician orders and Plan of Care to aid symptom control and maintain ADLs.
• Documents each visit in the patient’s clinical record and provides reports to the physician and administrator.
• Participates in the Interdisciplinary Group and contributes input to the Plan of Care.
• Instructs patient, family, and Interdisciplinary team on functional progress and plan adjustments.
• Prepares written discharge summaries when services end and forwards copies to the physician.
• Educates patients and caregivers in home therapy strategies as appropriate.
