Job summary

Active Generations is seeking a part‑time Caregiver Services Specialist in Sioux Falls, SD. You will educate and support caregivers, identify needs, and connect families with resources to plan for current and future care.

The role collaborates with staff to strengthen caregiver services, document interactions, and contribute to program improvement while delivering compassionate, person-centered support.

Responsibilities

• Provide individualized consultation, education, resource navigation, and support to caregivers and families, including assistance with immediate and long-term care planning.
• Identify caregiver needs and connect individuals with appropriate resources, referrals, respite services, support programs, and community-based services, including programming provided by Active Generations.
• Facilitate caregiver and grief support groups, family meetings, group coaching, and other caregiver education and support activities.
• Coordinate caregiver support services, including consultations, respite resources, educational programming, and other available services.
• Support evidence-based caregiver programs and interventions.
• Develop and maintain tools, guides, processes, and documentation standards that support consistent and effective caregiver services.
• Maintain accurate and confidential documentation of caregiver interactions, referrals, services, demographics, program participation, and outcomes.
• Gather caregiver feedback and use program data to identify opportunities to strengthen services, processes, and participant engagement.
• Perform additional duties as assigned by program management.
