Active Generations is seeking a part‑time Caregiver Services Specialist in Sioux Falls, SD. You will educate and support caregivers, identify needs, and connect families with resources to plan for current and future care.

Responsibilities
- Provide individualized consultation, education, resource navigation, and support to caregivers and families, including assistance with immediate and long-term care planning.
- Identify caregiver needs and connect individuals with appropriate resources, referrals, respite services, support programs, and community-based services, including programming provided by Active Generations.
- Facilitate caregiver and grief support groups, family meetings, group coaching, and other caregiver education and support activities.
- Coordinate caregiver support services, including consultations, respite resources, educational programming, and other available services.
- Support evidence-based caregiver programs and interventions.
- Develop and maintain tools, guides, processes, and documentation standards that support consistent and effective caregiver services.
- Maintain accurate and confidential documentation of caregiver interactions, referrals, services, demographics, program participation, and outcomes.
- Gather caregiver feedback and use program data to identify opportunities to strengthen services, processes, and participant engagement.
- Perform additional duties as assigned by program management.

As a Caregiver Services Specialist, you’ll provide education, resources, consultation, and ongoing support to caregivers and families while helping strengthen Active Generations’ Caregiver Services programs. This position works directly with caregivers to identify needs, navigate resources, plan for current and future care needs, and access appropriate support.
The Caregiver Services Specialist also supports program development, documentation, outcome tracking, staff education, and continuous improvement to ensure caregiver services remain responsive, effective, and aligned with participant needs.
Provide individualized consultation, education, resource navigation, and support to caregivers and families, including assistance with immediate and long-term care planning.
Identify caregiver needs and connect individuals with appropriate resources, referrals, respite services, support programs, and community-based services, including programming provided by Active Generations.
Facilitate caregiver and grief support groups, family meetings, group coaching, and other caregiver education and support activities.
Coordinate caregiver support services, including consultations, respite resources, educational programming, and other available services.
Support evidence-based caregiver programs and interventions.
Develop and maintain tools, guides, processes, and documentation standards that support consistent and effective caregiver services.

Associate’s or Bachelor’s degree in Social Work, Human Services, Gerontology, Sociology, Psychology, or a related field preferred.
Minimum one year of experience working with older adults, caregivers, individuals living with dementia-related conditions, human services, or a related area required.
Experience providing individual, family, or caregiver consultation and support preferred.
Experience facilitating support groups, educational programs, or evidence-based programming is a plus.
Strong interpersonal, communication, education, and relationship-building skills. Ability to establish trust and provide compassionate, participant-centered support to caregivers and families. Knowledge of or ability to learn resources and services available to caregivers, older adults, and individuals living with dementia or other chronic conditions.
Strong organizational, documentation, and problem‑solving skills with the ability to manage multiple participants, referrals, programs, and follow‑up responsibilities. Comfortable facilitating individual consultations, family meetings, support groups, and educational activities.
Ability to collect, organize, and interpret program information to support reporting, evaluation, and continuous improvement. Comfortable developing resources, procedures, training materials, and other tools that support consistent program delivery. Ability to collaborate effectively with team members, community organizations, and service providers while maintaining confidentiality and exercising sound professional judgment.
Regularly sit, stand, walk, bend, and reach throughout the workday.
Use a computer, telephone, mobile device, and other office equipment for extended periods.
Communicate effectively in person, by telephone, virtually, and electronically.
Navigate Active Generations facilities, community locations, meeting spaces, and other environments as needed.
Occasionally lift and carry program materials or supplies weighing up to 25 pounds.
Travel locally for participant meetings, community partnerships, programming, training, or other activities as needed.
This position is primarily performed in a professional, community-based environment and includes regular interaction with caregivers, families, team members, and community partners. Work may include individual consultations, family meetings, support groups, educational programs, and community-based activities. Schedules may vary based on participant, program, and organizational needs.
