Job summary
U.S. Department of Veterans Affairs in Durham, NC seeks a Caregiver Support Coordinator to lead the facility CSP, deliver evidence-based caregiver services, and provide education and consultation to caregivers, Veterans, and staff.
The role includes community outreach, resource development, and quality improvement activities. The CSC collaborates with interdisciplinary teams to ensure equitable access and high-quality caregiver support, performing home visits and coordinating education programs
Qualifications
- English proficiency required.
- Graduate of an accredited nursing program.
- Current RN license in US state or territory; eligibility for VA licensure.
- State licensure and bridge program pathways may apply.
- Clinical leadership and outreach experience helpful.
Responsibilities
- Assists in day-to-day Caregiver Support Program operations.
- Leads caregiver education, consultations, and staff training.
- Develops caregiver groups and educational resources.
- Collaborates with CSP team to ensure equitable access to services.
- Performs regular home visits and responds to caregiver inquiries.
