Qualifications

Responsibilities

• Provide direct care and supervision to residents per individualized care plans.
• Assist residents with ADLs including bathing, dressing, grooming, toileting, eating, and personal hygiene.
• Administer medications as prescribed and document accordingly.
• Assist with mobility, transfers, repositioning, and ambulation with safe techniques.
• Monitor changes in residents' physical/mental health and report concerns.
• Encourage participation in recreational, social, and therapeutic activities.
• Provide companionship and emotional support to enhance quality of life.
• Prepare nutritious meals and snacks per dietary needs and preferences.
• Maintain a clean, safe living environment and complete housekeeping tasks as required.
• Complete required documentation accurately including progress notes and logs.

Essential Duties and Responsibilities

• Provide direct care and supervision to residents in accordance with individualized care plans.
• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, eating, and personal hygiene.
• Support individuals experiencing behavioral health challenges by utilizing patience, empathy, de-escalation techniques, and trauma-informed care practices.
• Provide specialized care and support for residents living with dementia, Alzheimer's disease, memory loss, or cognitive impairments.
• Recognize and appropriately respond to behaviors associated with dementia, including confusion, wandering, agitation, sundowning, and memory-related challenges.
• Administer medications as prescribed and document medication administration according to established procedures.
• Assist with mobility, transfers, repositioning, and ambulation while following safe lifting and transfer techniques.
• Monitor residents for changes in physical, mental, emotional, or behavioral conditions and report concerns promptly.
• Encourage participation in recreational, social, and therapeutic activities that promote engagement and well-being.
• Provide companionship, emotional support, and meaningful interaction to enhance residents' quality of life.
• Prepare nutritious meals and snacks while adhering to dietary restrictions, physician orders, and resident preferences.
• Maintain a clean, safe, and comfortable living environment through housekeeping tasks, including laundry, cleaning resident areas, dishwashing, and waste disposal.
• Complete required documentation accurately and timely, including progress notes, incident reports, medication records, and communication logs.
• Maintain resident confidentiality and uphold resident rights at all times.
• Respond appropriately to emergencies and follow established safety procedures and protocols.
