Altra Homecare Group in Lafayette, IN is seeking a Personal Care Assistant to provide essential in-home care tailored to each patient’s needs. You will support daily living activities, help with mobility, monitor health, and document care under supervision.

This role requires at least 18 years old, a valid driver’s license and auto insurance, and ideally 1 year of related experience. Training compliance and reliable communication are essential, with a flexible, hourly schedule and benefits

Qualifications

• Valid driver's license and auto insurance.
• Preferred: 1 year of relevant work experience.
• Ability to read, follow written instructions and document care.
• Compliance with training requirements per State and Federal laws.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition to supervisor.
• Prepare light meals and assist with feeding.
• Monitor and record patient temperature.
• Assist patients with ambulation and ROM exercises.
• Maintain documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance and light housekeeping.
• Provide companion services including letter-writing and reading.

Responsibilities:

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition directly to their supervisor.
• Prepare light meals and assist with patient feeding when required.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct range of motion exercises and other simple physical activities.
• Maintain comprehensive documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
• Provide companion services, including letter-writing and reading.

Requirements:

• Must be 18 years of age or older.
• Possess a valid Driver's License and Auto Insurance.
