Altra Homecare Group in Lafayette, IN seeks a compassionate Personal Care Assistant to support patients at home. You will assist with daily living activities, monitor health, and follow care plans under supervision, ensuring safety and dignity in the patient’s own home.

The role requires at least 18 years old, a valid driver’s license with auto insurance, and reliable communication to document care, report changes, and coordinate with supervisors.

Qualifications

• Must be 18 years of age or older.
• Possess a valid Driver's License and Auto Insurance.
• Preferred: Minimum of 1 year of relevant work experience.
• Ability to read, follow written instructions, and document care provided.
• Comply with training requirements as per State and Federal laws.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition directly to their supervisor.
• Prepare light meals and assist with patient feeding when required.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct range of motion exercises and other simple physical activities.
• Maintain comprehensive documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
• Provide companion services, including letter-writing and reading.
