What You Will Do:

You will be responsible for implementing customized care plans that support the whole person—mind, body, and spirit. You will treat clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care to clients in their homes
• Execute personalized weekly schedules and care plans
• Complete daily tasks safely and professionally (ADLs)
• Provide personal companionship and ongoing social & emotional support
• Assist with personal care activities including hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment at all times
• Provide transportation to appointments as needed
• Document task completion and report changes in physical or mental condition
