What You Will Do

You will be responsible for implementing customized care plans that promote the whole person—mind, body, and spirit. Your role involves treating clients like family and going above and beyond to enhance their quality of life.

Essential Functions

• Deliver exceptional care to clients in their homes.
• Help execute personalized weekly schedules and care plans.
• Complete daily tasks safely and professionally (ADLs).
• Provide personal companionship and social/emotional support.
• Assist with personal care activities such as hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep.
• Maintain safety, order, and client enjoyment at all times.
• Provide transportation to appointments as needed.
• Document task completion and report changes in client’s condition or behavior.
