Altra Homecare Group in Indiana seeks a compassionate Personal Care Assistant to join our home-based care team in Frankfort. You will provide personalized support to patients in their own homes, delivering daily living assistance with dignity and empathy in a community-focused agency.

Responsibilities include assisting with laundry, meals, mobility, medication reminders, documentation, and safety checks; you’ll communicate changes to the supervisor, follow care protocols, and maintain privacy

Qualifications

• Must be 18 years or older.
• Valid driver's license and auto insurance.
• Preferred: Minimum of 1 year of relevant work experience.
• Ability to read, follow written instructions, and document care provided.
• Comply with training requirements as per State and Federal laws.

Responsibilities

• Assist with patient laundry when necessary.
• Provide medication reminders.
• Ensure patient safety.
• Report significant changes in the patient's condition directly to their supervisor.
• Prepare light meals and assist with patient feeding when required.
• Monitor and record patient temperature.
• Assist patients with ambulation.
• Conduct range of motion exercises and other simple physical activities.
• Maintain comprehensive documentation and adhere to care protocols.
• Follow privacy policies and infection control guidelines.
• Offer bathing assistance.
• Perform light housekeeping.
