Job summary

Community Horizons is seeking a compassionate caregiver to assist a disabled young adult at home. The role emphasizes building meaningful connections and providing hands-on support with daily care needs during a 4pm–9pm shift on a rotating 2-week schedule.

Responsibilities include full-care assistance with transfers using a Hoyer lift, bathing, dressing, toileting, hygiene, meal support, mobility aid, light companionship, and routine care throughout the shift.

hours are on a rotating 2- week schedule. the hours are 4pm-9pm but can vary slightly depending on the day of the week.
