LiveWell Homecare Agency is seeking a compassionate Caregiver to provide hands-on support in residents' homes. You will assist with ADLs, follow individualized service plans, document changes, and promote safety, dignity, and independence through meaningful interactions.

Responsibilities

• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, mobility, and personal care per individualized plans.
• Document changes and progress in resident plans and records.
• Promote safety, dignity and independence while engaging residents in conversations and activities.
• Support dining services and maintain a clean, welcoming environment.
• Create enriching experiences for memory care residents and support their quality of life.
