What You Will Do:

You will help implement customized care plans that support the whole person—mind, body, and spirit. You will treat clients like family and go above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care in clients' homes
• Assist with personalized weekly schedules and care plans
• Perform daily tasks safely and professionally (ADLs)
• Provide companionship and social/emotional support
• Assist with personal care, mobility, incontinence, light housekeeping, feeding, and meals
• Maintain safety, order, and client enjoyment
• Transport clients to appointments as needed
• Document tasks and report changes in clients' conditions
