What You Will Do:

Implement customized care plans that support the whole person—mind, body, and spirit. Treat clients like family and go above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care in clients' homes.
• Execute personalized weekly schedules and care plans.
• Perform daily tasks safely and professionally (ADLs).
• Provide companionship and ongoing social and emotional support.
• Assist with personal care activities, mobility, incontinence care, light housekeeping, feeding, and meal prep.
• Maintain safety, order, and client enjoyment.
• Transport clients to appointments as needed.
• Document tasks completed and report changes in clients' conditions.
