What You Will Do:

You will have the ultimate responsibility to help implement customized care plans that encompass solutions to nurture the whole person in mind, body, and spirit. You will treat your clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Delivering exceptional care to clients in their homes
• Helping execute comprehensive personalized weekly schedules and care plans
• Completing daily tasks safely and professionally (ADLs)
• Providing personal companionship and ongoing social & emotional support
• Assisting with personal care activities including hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintaining safety, order, and client enjoyment at all times
• Providing transportation to and from appointments as needed
• Documenting task completion and reporting changes in clients' physical or mental condition
