What You Will Do:

Implement customized care plans that support the whole person—mind, body, and spirit. Treat clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care in clients' homes
• Assist with personalized weekly schedules and care plans
• Perform daily tasks safely and professionally (ADLs)
• Provide companionship and emotional support
• Assist with personal care, mobility, incontinence, light housekeeping, and meals
• Maintain safety, order, and client enjoyment
• Transport clients as needed
• Document tasks and report changes in client condition
