Hartford HealthCare is seeking a Homemaker Caregiver to support elderly and disabled clients in their homes in Connecticut. You will provide personal hygiene assistance, meal preparation, and help with daily activities, while coordinating with nurses and care coordinators to ensure a safe, dignified plan of care.

The role emphasizes holistic, client-centered care, recording each client's progress on the care plan, and offering emotional support to clients and families.

Qualifications

• Experience in elder care or home health care preferred.
• Ability to assist with activities of daily living and personal hygiene.
• Reliable, compassionate, good communication with clients and families.

Responsabilités

• Personal hygiene and grooming care to clients
• Assistance with toileting, ambulation, etc.
• Providing meals for clients' nutritional needs
• Reporting changes in clients' condition to nurse, supervisor, or client care coordinator
• Performing routine housekeeping and home management duties
• Recording each client's care on care plan and submitting care plan in a timely manner
• Assisting with providing emotional support for the clients and their families

Shift Detail: Hourly cases range anywhere from 4-12 hours at any time in the day, evening or night. Shifts will take place within 30 minutes of where you reside.
