Job summary

Ascend Health Idaho is seeking a compassionate Certified Nursing Assistant (CNA) to join our Home Health and Hospice team in Idaho. You will provide direct care, companionship, and support to patients in their homes, following the plan of care set by the clinical team.

Ideal candidates are patient, dependable, and skilled communicators, with a valid Idaho CNA certification and a driver’s license. This part-time position offers the opportunity to make a meaningful difference in patients’ lives

Qualifications

• Current CNA certification in Idaho.
• Home health/hospice experience preferred.
• Compassionate, patient, reliable with strong communication skills.
• Valid driver's license, reliable vehicle, current auto insurance.

Responsibilities

• Provide personal care, companionship, and support to patients in their homes.
• Assist with activities of daily living (bathing, dressing, grooming, feeding, mobility, toileting).
• Monitor and report patient condition changes to the supervising nurse.
• Provide emotional support to patients and families.
• Document all patient care and maintain accurate, timely records in the EMR.
• Follow the plan of care as directed by the clinical team.

Ascend Health is looking for a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Home Health and Hospice team. As a CNA with Ascend, you will play a vital role in providing hands-on care and support to patients and families in the comfort of their homes.
