Middlefield Oaks Memory Care in Cottage Grove, OR is seeking compassionate Care Partners for a part-time Memory Care team. Shifts include Day, Evening, and Night; weekend availability is required. Responsibilities

• Provide assistance with activities of daily living per person-centered plan.
• Assist with bathing, grooming, dressing, ambulation, meals, and linen changes.
• Support residents with daily requirements and tasks as requested.
• Ensure residents have privacy, respect, and dignity.
• Follow infection control procedures and safety protocols.
• Complete state-required training at hire and annually.

Middlefield Oaks Memory Care, located in Cottage Grove, OR is seeking a compassionate Caregivers to join our Memory Care team on a part-time basis. Applicants must be available to work weekends and be open to working day, evening, or overnight shifts as scheduled.

Responsibilities of the Care Partner

• Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.
• Provides personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.
• Assist in daily requirements and tasks as requested.
• Ensure residents are provided privacy, respect, and dignity.
• Follows standard precautions and infection control procedures.
• Completes state required training upon hire and annually.

Required Experience

• Possess, at a minimum, a high school diploma or equivalent.
• Be at least 18 years of age.
• We welcome applicants with prior experience in caregiving, home care, or community-based care.
• Basic computer skills.
• Must be able to speak, read, and write in the English language.
