Job summary

TEEMA is seeking a dedicated Home Health Aide/Certified Nursing Assistant to provide compassionate in-home or center-based care for older adults and frail individuals. You will follow individualized care plans, perform daily personal care tasks, and document care activities in designated forms or electronic health records.

This on-site, full-time role runs Monday through Friday, 8:00 AM to 4:30 PM, within day center settings.

Qualifications

  • Minimum 1 year of experience with the elderly or frail population.
  • Ability to work independently and as part of a team.
  • Flexibility to work in home health or center-based settings.
  • A positive, compassionate attitude with patient-centered care.
  • Basic conversational English; bilingual may be required.
  • Ability to translate or interpret basic information (testing may be required).

Responsibilities

  • Provide personal care and escort assistance per patient care plan (grooming, dressing, bathing, toileting, peri-care, housekeeping, meal preparation/feeding, laundry).
  • Observe and report changes in patient condition to the team.
  • Demonstrate safe practices by using proper body mechanics and maintaining a safe environment.
  • Interact with patients, families, and the team in a positive, professional, and supportive manner; encourage engagement in conversations and activities.
  • Perform additional related duties as assigned.

Job description

Duties

Provide personal care and escort assistance based on each patient’s individualized care plan, including grooming, dressing, bathing, toileting, peri-care, housekeeping, meal preparation/feeding, laundry, and related tasks.

Observe and report changes in a patient’s condition to the team to support coordinated and effective care.

Demonstrate safe practices by using proper body mechanics, following universal precautions, and maintaining a safe environment.

Interact with patients, families, and the team in a positive, professional, and supportive manner; encourage engagement in conversations and activities.

Perform additional related duties as assigned.

Required Qualifications

Minimum 1 year of experience working with the elderly or frail population.

Ability to work both independently and collaboratively as part of a team.

Flexibility to work in different care environments, including in-home or center-based settings.

A positive, compassionate, and conscientious attitude with a strong commitment to patient-centered care.

Basic conversational English skills; testing may be required.

Bilingual skills may be required depending on patient needs; language testing may be required.

Ability to translate or interpret basic information for supervisors, patients, and fellow caregivers (testing may be required).

Ability to document care-related information in basic English using designated forms or electronic health records.

Desired Qualifications

Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification

Functional Requirements & Working Conditions

Ability to stand, reach, walk, push, pull, lift, grasp, talk, hear, and perform repeated wrist/hand motions.

Must have sufficient visual acuity.

Must be able to safely lift or move weights within personal physical ability, using lifting devices or requesting assistance when needed.

Location & Work Type

On-site role providing services in the day center settings. May require flexibility to work across multiple care locations.
