Job summary

Sharecare, Inc. through CareLinx by Sharecare is hiring for an In-Home Caregiver in Sandwich, MA. This part-time, on-site role provides in-home non-clinical care to health plan members and private clients.

You’ll use the CareLinx app to schedule, document, and communicate with the CarePartner team, selecting shifts that fit your availability. The role emphasizes compassionate support, daily living assistance, and documenting tasks accurately.

Qualifications

• Minimum 6 months of caregiving experience or relevant education/certification (CNA, HHA, MA, or equivalent).
• Access to a smartphone capable of running the CareLinx app for scheduling and documentation.
• Ability to communicate in English; bilingual candidates encouraged.
• Reliable transportation to client homes and ability to pass a background check.
• Program-specific trainings and medical clearance may be required.
• Valid driver’s license and MVR check for driving shifts.

Responsibilities

• Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, non-clinical medication reminders, and light housekeeping.
• Complete and document all care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes in a client’s condition to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with your CarePartner regarding any case updates or client changes.
• Ensure patient privacy, dignity, and well-being in a safe and secure environment.
• Maintain client confidentiality at all times.
• Additional shift opportunities (based on client needs): transportation and errand support.
• Hospital discharge escort — safely accompanying members home after discharge.
• Telehealth navigation — helping members connect with virtual doctor visits.

WHAT YOU'LL DO

WHAT WE'RE LOOKING FOR

Required qualifications:

Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)

Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)

Ability to communicate in English (bilingual caregivers strongly encouraged to apply)

Reliable transportation to and from client homes

Must pass a background check

Program-specific requirements (may apply depending on assignment):

Completion of program-specific training prior to accepting shifts

Medical clearance such as TB test, physical exam, or drug screening

Valid driver's license and MVR check if accepting driving shifts

Additionaldocumentation may berequiredbased on state law or client contract
