CareLinx by Sharecare is hiring in Palm Coast, FL to provide non-clinical in-home care as an in-home caregiver. The role offers flexible part-time shifts through the CareLinx app and matches to client needs based on your availability.

Qualifications

• Minimum 6 months of caregiving experience or relevant education/certification (CNA, HHA, MA, or equivalent).
• Access to a smartphone capable of running the CareLinx app.
• Ability to communicate in English; bilinguals encouraged.
• Reliable transportation to client homes and ability to pass a background check.
• Program-specific trainings and state requirements may apply.

Responsibilities

• Assist clients with activities of daily living (ADLs) and provide companionship.
• Meal prep, mobility support, bathing, grooming, dressing, and medication reminders (non-clinical).
• Document care tasks accurately in the CareLinx app during each shift.
• Monitor and report changes in client condition to the CareLinx clinical team.
• Submit timesheets on time through the app.
• Maintain patient privacy and dignity in a safe environment.
• Transport support, discharge escorts, and telehealth navigation as needed.

WHAT YOU'LL DO

Core responsibilities for all caregivers:

Assistclients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping

Complete and document all care tasks accurately in theCareLinxmobile appduringeach shift

Monitor and report changes in a client’s condition or self-care abilities to theCareLinxclinical team

Submit timesheets on time through theCareLinxapp

Communicate with your CarePartnerregardingany case updates or client changes

Ensure patient privacy, dignity, and well-being in a safe and secure environment

Maintain client confidentiality at all times

Additionalshift opportunities (based on client needs):

Hospital discharge escort — safelyaccompanyingmembers home after discharge

Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required qualifications:

Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)

Accessto a smartphone capable of running theCareLinxapp (required for scheduling, documentation, and timesheets)

Ability to communicate in English (bilingual caregivers strongly encouraged to apply)

Reliable transportation to and from client homes

Must pass a background check

Program-specific requirements (may apply depending on assignment):

Completion of program-specific training prior to accepting shifts

Medical clearance such as TB test, physical exam, or drug screening

Valid driver's license and MVR check if accepting driving shifts

Additionaldocumentation may berequiredbased on state law or client contract
