Job summary

CareLinx by Sharecare is hiring experienced in-home caregivers to support health plan members and private clients in Hemet, CA. You’ll manage non-clinical tasks via the CareLinx app and choose your hours, enjoying steady shifts or per-diem flexibility while we handle the matches.

Required: 6 months of caregiving experience or CNA/HHA/MA certification, English communication, reliable transportation, and the ability to pass a background check.

• Access to a smartphone capable of running the CareLinx app.
• Ability to communicate in English (bilinguals encouraged).

Responsibilities

• Assist clients with activities of daily living (ADLs) such as companionship, meal prep, mobility, bathing, grooming, and dressing.
• Complete and document all care tasks accurately in the CareLinx app during each shift.
• Monitor and report changes in a client’s condition to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with your CarePartner regarding case updates or changes in client needs.
• Maintain patient privacy, dignity and well-being in a safe environment.

WHAT YOU'LL DO

Core responsibilities for all caregivers:

Assistclients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping

Complete and document all care tasks accurately in theCareLinxmobile appduringeach shift

Monitor and report changes in a client’s condition or self-care abilities to theCareLinxclinical team

Submit timesheets on time through theCareLinxapp

Communicate with your CarePartnerregardingany case updates or client changes

Ensure patient privacy, dignity, and well-being in a safe and secure environment

Maintain client confidentiality at all times

Additionalshift opportunities (based on client needs):

Transportation and errand support

Hospital discharge escort — safelyaccompanyingmembers home after discharge

Telehealth navigation — helping members connect with virtual doctor visits

WHAT WE'RE LOOKING FOR

Required qualifications:

Minimum 6 months ofcaregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)

Accessto a smartphone capable of running theCareLinxapp (requiredfor scheduling, documentation, and timesheets)

Ability to communicate in English (bilingual caregivers strongly encouraged to apply)

Reliable transportation to and from client homes

Must pass a background check

Program-specific requirements (may apply depending on assignment):

Completion of program-specific training prior to accepting shifts

Medical clearance such as TB test, physical exam, or drug screening

Valid driver’s license and MVR check if accepting driving shifts

Additionaldocumentation may berequiredbased on state law or client contract
