Responsibilities

• Behavioral Health Support: engage clients and provide directed interventions.
• Resource Coordination: connect clients to housing, health, and services.
• Documentation & Billing: maintain case notes and support billing targets.
• Crisis Intervention & Support: provide immediate crisis response.
• Ongoing Support & Advocacy: monitor progress and advocate for housing and services.
• Collaboration & Coordination: work with clinical teams on care planning.
• Community Resource Knowledge: stay updated on local resources.
• Flexible to work varied hours including evenings/weekends.
