Job summary

Workforce Connections, Inc. is seeking a Career Planner to provide person-centered case management and employment planning services that help individuals and families achieve employment, economic stability, and long-term self-sufficiency.

You will assess strengths and barriers, develop individualized employment plans, coordinate supportive services, and connect participants with resources. You will work closely with participants, employers, and community partners to expand opportunities,

Qualifications

Bachelor's degree in human services, education, business, or related field preferred; substitutions may be considered.
Experience in case management, employment services, workforce development, or related human services preferred.

Responsibilities

Provide intake, eligibility, case management, and employment planning services.
Develop individualized employment and service plans for participant success.
Connect participants with employment, education, training, and community resources.
Build partnerships with employers, educational institutions, and community organizations.
Maintain documentation and ensure program compliance.
Manage caseload and monitor participant progress and outcomes.
Perform other duties as assigned.
