Responsibilities

• Manages a caseload of participants and provides counseling and mentoring.
• Assesses participant competencies, work history, education and skills; identifies challenges and solutions.
• Conducts regular meetings with participants for job search, education, job retention, and readiness.
• Creates an individualized overview of services and employment plans.
• Maintains information about area resources and employers.
• Tracks attendance and employment retention goals.
• Notifies government agencies of lost or reduced hours.
• Maintains regular contact with participants across locations to address progress.
• Provides information on training and jobs leading to advancement.
• Monitors progress toward self-sufficiency across the program cycle.
• Prepares and maintains accurate participant files (electronic and paper).
• Delivers case management focused on preparing for employment and self-sufficiency.
• Facilitates access to training, education, and employment services; provides level-based case management.

Specific Job Functions

• Manages a caseload of participants and provides counseling and mentoring
• Assesses participant competencies, work history, educational attainment, skills and abilities; identifies challenges to finding employment and prompts them to find solutions
• Conducts regular ongoing individual meetings with participants for job search, education, job retention, and/or job readiness
• Completes an individualized overview of available services and works with participant to create an employment plan for short- and long-term goals
• Maintains information about area resources and employers
• Ensures participant accountability and attendance; tracks and maintains employment retention goals
• Provides government agencies with prompt notification that a participant has lost or reduced hours of employment
• Maintains scheduled, periodic contact with participants in a variety of locations to assess job retention/advancement issues
• Provides information to participants on available training and/or jobs that will lead to advancement
• Assumes central responsibility for participants achieving self-sufficiency by monitoring progress throughout the program cycle, beginning with referral and continuing through retention and advancement efforts
• Prepares, organizes, and maintains accurate, updated information in both electronic and paper participant files reflecting the entire history of a program participant, including a log of supportive services issued
• Provides case management with a focus on helping customers to prepare for employment that leads to self-sufficiency
• Facilitates customer access to training, education, and employment services, as well as job-specific information; provides case management to customers at the appropriate level

Qualifications

• Ability to maintain flexibility as needs of contract or contractor require
• A working knowledge of MS Office and Google docs
• Two years of experience in career advisement and development, workshop facilitation, developing workforce curriculum, public speaking and content development
• Ability to prioritize tasks and document visits and contacts
• Demonstrated telephone, computer and strong written and verbal communication skills
• Strong leadership skills with a demonstrated ability to thrive in environments with multiple stakeholders, frequent change, and diverse objectives
• Professional understanding of a vast array of occupations and career paths, ideally including those in the healthcare field
• Preferred Bachelor's degree from an accredited college or university in education, social services, healthcare and/or communications
• Customer service experience required
