Job summary

Qualifications

• High School Diploma required.
• Associate Degree preferred.
• Prior experience in a healthcare setting.
• Experience in discharge planning, billing utilization review, financial counseling or registration preferred.
• Ability to develop and maintain good working relationships.
• Ability to work in a high-volume environment while maintaining quality.
• Strong analytical and problem-solving skills.
• Excellent written and verbal English communication skills.
• Ability to manage workload and priorities with minimal supervision.
• Ability to handle complex problems and emergent situations.
• Secretarial experience and customer service skills.
• Microsoft Word and 30 WPM typing required.

Responsibilities

• Coordinate post-discharge referrals to ensure continuity of care.
• Interact frequently with care managers, patients, payers, and community care providers.
• Data entry and analysis related to insurance coverage and resources.
• Coordinate post-discharge services including home health, equipment, nursing homes, transfers, and transportation.
• Conduct post-discharge follow-up calls to verify services and resolve issues.
• Elicit feedback from patients/families and report dissatisfaction for service recovery.
