Job summary

Baptist Health is seeking an experienced Case Manager to join the care coordination team at our Florida facilities. You will supervise the care coordination staff, optimize resource use, and ensure high-quality patient care from admission to discharge.

Minimum 3 years of case management experience, BSN, and active licenses/certifications are required. You will work with physicians and care teams to support reimbursement and discharge planning, with a mission-driven, faith-based environment.

Qualifications

• Knowledge of reimbursement principles and discharge planning.
• Advanced written, verbal and interpersonal skills.

Responsibilities

• Collaborates with health care delivery team to enhance patient care from admission to discharge.
• Lead supervisor of the care coordination team to ensure high-quality care and appropriate length of stay.
• Adjust staff assignments to assure coverage and optimize resource utilization.
• Serve as a resource for utilization management issues and reimbursement for services.
