Job summary

Lake Region Healthcare Corporation is seeking a Care Transitions Registered Nurse (RN) to coordinate patient care from hospital/ED to home or another facility, aiming to lower readmissions and ensure safe transitions. You will collaborate with patients, families, providers, and community resources.

Responsibilities include assessment, discharge planning, care coordination, and patient/family education. Hours are 80 biweekly, with day shifts, and a competitive hourly wage.

Qualifications

• Active, unrestricted RN license in the applicable state.
• ADN or BSN from an accredited program.
• CTC or case management certification preferred.

Responsibilities

• Assess high-risk patients to guide discharge planning.
• Coordinate care between inpatient and outpatient teams.
• Educate patients and families on disease management and follow-up.
• Support safe transitions of care to reduce readmissions.
