Job summary

Methodist Southwest Medical Center in Dallas, TX, is seeking a Care Transitions Navigator to coordinate discharge planning and patient throughput, promoting high-quality outcomes while balancing resource use. You will identify barriers to throughput and minimize delays in discharge planning.

The role requires a Social Work degree (MSW preferred) or RN with BSN, Texas licensure (LMSW/LBSW or RN) preferred, and at least 1 year of related experience.

Qualifications

• Bachelor’s degree in Social Work, Master’s degree in Social Work, Registered Nurse with BSN preferred.
• LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferred.
• 1 Year related work experience.
• Ability to prioritize multiple tasks in a fast-paced work environment.
• Ability to periodically flex work schedule as indicated by client or hospital needs.
• Ability to develop and maintain good working relationship with all levels of staff.
• Ability to communicate clearly, both verbally and in writing, with empathy, flexibility, and objectiveness, and maintains a professional approach to handling confidential information.
