Job summary

The Case Management Coordinator oversees post-discharge transitions and follow-up for members with chronic conditions, coordinating care with primary care providers, specialists, and community resources to prevent hospital readmissions and improve outcomes.

The role requires evaluating patient needs, creating individualized care plans, and educating patients and caregivers using motivational interviewing techniques

Qualifications

• Proven experience in clinical case management, care coordination, utilization review, or population health management within a healthcare setting.
• CPR/BLS for Healthcare Providers is required.
• Bilingual English/Spanish/Creole preferred to serve diverse member populations.

Responsibilities

• Manage post-discharge transition and follow-up for members, coordinating care with providers and community resources.
• Develop individualized care plans and educate patients and caregivers on self-management and red-flag symptoms.
• Document all interactions and interventions in the care management system in a timely and compliant manner.
