Job summary

Providence at Home with Compassus is seeking a Care Partner to coordinate safe, efficient transitions of care for hospitalized patients and evaluate post-acute home-based services. This in-person role engages with physicians, case management, families, and post-acute providers to improve outcomes and reduce readmissions.

You will lead discharge planning workflows, develop individualized transition plans, and arrange post-acute services while ensuring timely and accurate handoffs to care teams.

Qualifications

• Associate's degree required.
• Bachelor's degree preferred.
• 2–3 years healthcare setting experience preferred (hospital/long-term care).
• Liaison, care coordination, partner, and discharge planning experience beneficial.

Responsibilities

• Evaluate patients for home-based and post-acute care services based on clinical factors.
• Review inpatient referrals and prioritize patients using clinical judgment and analytics tools.
• Coordinate discharge planning and transition-of-care plans with physicians and care teams.
• Arrange post-acute services including home health, hospice, equipment, medications, and follow-up care.
• Ensure accurate handoffs to post-acute providers and maintain referral documentation.
