• Bachelor's degree in Social Work, Psychology, or related field.
• 1–2 years of experience in a skilled nursing facility with Medicare/Medicaid populations.

Responsibilities

• Reviews and documents benefit, medical, social and financial information accurately.
• Coordinates discharge planning with the interdisciplinary team.
• Performs MDS interviews and supports care planning.
• Advocates for residents and liaises with family.
• Maintains documentation on discharge status and authorizations.
