Responsibilities

Assist patients in navigating post-acute care, aiming to improve patient outcomes and referral satisfaction. Develop patient-centered transition plans using evidence-based guidelines, protocols, and metrics. Represent the organization in strategic relationships with health systems, hospitals, physicians, and executive opportunities.

Qualifications

Essential Education and Experience

• Graduate of an approved nursing, therapy, or social work program
• Licensed in the practicing state
• Two years of demonstrated field experience
• Knowledge of laws and regulations governing Medicare-certified home health and hospice
• Basic technology skills, including mobile device operation

Preferred Education and Experience

• Registered nurse or physical therapist preferred
• Three years of field experience preferred
• Experience in home health or healthcare sales preferred

Requirements

• Valid driver’s license
• Automobile liability insurance
• Dependable transportation
• Ability to drive safely in all weather conditions
