Position Overview

As a Home Care Coordinator, you’ll play a vital role in advancing Trinity Health at Home’s mission by facilitating seamless, patient-centered care transitions. You’ll be responsible for coordinating and enhancing the referral process, supporting patients as they transition from acute care settings to post-acute home care. This role ensures patients experience smooth, cost-effective, and results-oriented care.

Employment Details

Type: Part-time (24 hours, Tues-Wed-Thursday)

Shift: Part time - 24 hours- Tues-Wed-Thursday

Qualifications

• Outstanding communication and customer service skills
• Must be RN/LPN/MSW or Therapist
• Strong critical thinking and problem-solving abilities
• Familiarity with Medicare regulations and managed care systems
• Proficiency in multitasking across various computer systems
• Current Ohio registration or licensure (preferred)
