Job summary

Compassus Providence at Home in Torrance, CA seeks a Care Partner to coordinate safe, efficient transitions of care for hospitalized patients and evaluate post-acute home-based care options in collaboration with physicians, case management, families, and post-acute providers.

This in-person role focuses on improving outcomes, reducing length of stay and readmissions, and ensuring patients receive the right care in the right setting at the right time.

Qualifications

• Associate's degree required.
• Bachelor's degree preferred.
• 2–3 years of healthcare experience preferred.
• Liaison, care coordination, discharge planning experience.
• Fluent in English with strong written and verbal communication.

Responsibilities

• Referral Evaluation & Clinical Assessment of home-based and post-acute services.
• Discharge Coordination & Care Transitions with hospital teams.
• Educate patients and families on post-acute options and expectations.
• Develop and implement transition-of-care plans aligned with goals.
• Documentation of referrals and care coordination in EHR.
• Support performance improvement and care network strategy.
