Job summary

Compassus is seeking a Care Partner to coordinate safe, efficient, patient-centered transitions of care for hospitalized patients and evaluate eligibility for home-based post-acute services. You will support discharge planning with physicians, case management, patients, families, and post-acute providers.

This in-person role emphasizes bedside engagement, collaboration, and ensuring patients receive the right care in the right setting at the right time.

Qualifications

• Associate's degree required.
• Bachelor's degree preferred.
• None required; 2–3 years healthcare experience preferred.

Responsibilities

• Evaluate patients for home-based and post-acute care services.
• Coordinate hospital discharge planning with care teams.
• Educate patients and families on post-acute options.
• Maintain relationships with physicians and discharge planners.
• Document care coordination activities in the EMR.
• Participate in rounds and care coordination meetings.
