Job summary

Corewell Health is seeking a Post-Acute Insurance Benefits Consultant to support the South Hospitals in Michigan. You will provide consultative and educational services to patients, families, physicians, care managers, and post-acute providers, focusing on insurance coverage, levels of care, and service availability within the service area.

The role emphasizes education, benefit verification, patient placement, and collaboration with the Marketing/Communications team to improve patient outcomes

Qualifications

• Associate's Degree related field required.
• Minimum 2 years of clinical experience in long-term, sub-acute, neuro or rehabilitative care.
• Experience with insurance benefits, coverage verification, and patient placement.

Responsibilities

• Provide consultative, educational support to patients, families, physicians and care teams about post-acute insurance benefits.
• Facilitate verification of benefits and assist in placing patients in appropriate post-acute settings.
• Educate internal and external stakeholders on post-acute services and coverage options.
• Collaborate with care managers and providers to coordinate patient discharges to post-acute providers.
• Support marketing/communications efforts and act as a resource to partner organizations.

Job description

This position will support Corewell Health South Hospitals as needed

Provides consultative, facilitative, technical and educational support to patients, families, physicians, care managers, finance departments and health care facilities with regards to post-acute insurance benefits and coverage and the types, appropriateness, levels-of-care and availability of local post-acute services in the service area. Provides formal and informal education, formal and informal consultative support, facilitates benefit verification and, assists customer groups in the placement of patients in the appropriate post acute setting. Serves as a resource to the Spectrum Health Hospital Group, Spectrum Health Medical Group, Priority Health, Medical Staff, post-acute providers, vendors and the larger community. The consultant may provide coverage for, and be deployed in support of, Post Acute Community Liaisons.

Essential Functions

• Serves as a post-acute services resource to the Spectrum Health Hospital Group, Spectrum Health Medical Group, Priority Health, vendors, post-acute providers, hospitals, Medical Staff and the larger community.
• Provides formal and informal education (CME, ANA or equivalent certified when relevant) regarding: nature, type and availability of post-acute services; best-practice clinical models; post-acute services role in improving patient outcomes and; role and value of post-acute providers in improving hospital, physician and community patient care metrics.
• Works closely with customer groups (hands-on when necessary) to help them understand, verify, qualify and quantify post-acute insurance and benefits coverage.
• Facilitates patient discharges to post-acute providers.
• Works closely with targeted Health Care Providers to help them understand which post-acute services will be of most benefit to their patients.
• Works with the manager to formulate and continually update provider lists.
• May provide coverage for, and be deployed in support of, Post Acute Community Liaisons.
• Is an integral part of the Marketing/Communications team and participates in such strategies that impact communication with the physician community.

Qualifications

Required

• Associate's Degree related field
• 2 years of relevant experience Clinical experience working with persons requiring long-term,sub-acute care, long-term care, neuro and rehabilitative services, home health, acute care or IRU.

Preferred

• Bachelor's Degree related field
• Relevant experience working with insurance companies and government payers
• Infusion therapy, home care and/or hospice experience
