Job summary

Sun Behavioral Health in Columbus seeks a Discharge Planning Coordinator to coordinate and implement case management strategies from intake to discharge. You will work with physicians, nurses, social workers and the intake department to ensure a seamless discharge process and high-quality patient care.

The role requires collaboration across teams, adherence to clinical guidelines, and active participation in safety and performance improvement activities.

Qualifications

• Proven ability to collaborate with physicians, nurses, therapists and social workers to coordinate discharge planning.
• Ability to update and implement care plans in collaboration with the healthcare team.
• Adherence to HIPAA and patient confidentiality standards in all documentation.
• Experience with performance improvement activities and clinical guidelines implementation.

Responsibilities

• Evaluate patient care plans and update as needed.
• Coordinate discharge plans within 72 hours of admission.
• Communicate aftercare needs to referrals within 24 hours of discharge.
• Assist Utilization Management and participate in safety and quality initiatives.
• Engage with family and community resources to meet post-discharge needs.
• Maintain accurate documentation in the medical record.
