Job summary

University Behavioral Center, part of Universal Health Services, is seeking a Discharge Planner to coordinate discharge planning for inpatients and transition patients to community care. This per-diem role offers weekdays and weekends opportunities and supports safe, timely transitions.

The ideal candidate holds a Bachelor’s in social work or related field and has experience in inpatient/outpatient mental health settings, with knowledge of discharge planning processes and VA systems preferred.

Qualifications

• Bachelor’s degree in social work or a related field is required.
• Experience in inpatient or outpatient mental health settings preferred.
• Court filing experience preferred; knowledge of VA systems, discharge planning, and case management.

Responsibilities

• Assists Social Services with discharge planning, including aftercare arrangements and referrals.
• Assists with completion of patient satisfaction surveys.
• Initiates follow-up calls to external referral sources and manages care point alerts.
• Tracks discharge data and compiles discharge reports; audits and confirms discharge paperwork.
